New patient form updates can be done through the update feature if the case is still live on the dashboard.
Any continued issues accessing historical data should be escalated to the SMART Helpdesk
Retrospective data can also be downloaded via the reporting function.
Please go back to the form and using the update feature beside the relevant field re-open the field. Emend the data and press save.
Data can be viewed within the data collection form itself prior to pressing save.
